Click on “Psychiatric Nurse Practitioner New Patient Paperwork” if this is your first appointment with our nurse practitioner, even if you’ve been seen before for counseling. Please download, fill out the .pdf, and either attach to an email (info@counselingassociatesllc.com) or print and bring to your appointment. Please arrive fifteen minutes prior to your first appointment.
